Улучшить результаты панкреатодуоденальной резекции определением оптимального реконструктивного этапа на основании объективной оценки функционального состояния верхнего отдела желудочно-кишечного тракта с помощью модифицированного “двойного” сцинтиграфического исследования.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. С 2016 по 2022 г. выполнено 147 панкреатодуоденальных резекций. На 30-е и 90-е сутки после операции больным выполняли “двойное” сцинтиграфическое исследование. Средний возраст пациентов составил 59,6 (23–83) года, соотношение мужчин и женщин – 1,27 : 1. Всем больным выполнили гастропанкреатодуоденальную резекцию или панкреатодуоденальную резекцию с сохранением привратника. По варианту реконструктивного этапа операций пациентов разделили на 3 группы. В 1-й группе выполняли реконструкцию по Чайлду, во 2-й группе – реконструкцию по Чайлду и формирование межкишечного анастомоза по Брауну, в 3-й группе реконструктивный этап осуществляли на выключенной по Ру петле тощей кишки.</p></sec><sec><title>Результаты</title><p>Результаты. В результате проведения “двойного” сцинтиграфического исследования наилучший результат на 30-е и 90-е послеоперационные сутки был отмечен у пациентов, перенесших панкреатодуоденальную резекцию с сохранением привратника, реконструктивным этапом по Чайлду с формированием межкишечного анастомоза по Брауну.</p></sec><sec><title>Заключение</title><p>Заключение. “Двойное” сцинтиграфическое исследование позволяет объективно определить моторно-эвакуаторную функцию желудка, билиодигестивного анастомоза и моторику тонкой кишки. На основании этого был определен оптимальный вариант реконструкции после панкреатодуоденальной резекции у больных с новообразованиями головки поджелудочной железы, периампулярной области и хроническим панкреатитом.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To improve the outcomes of pancreaticoduodenal resection by determining the optimal reconstruction based on objective assessment of the functional state of the upper gastrointestinal tract using a modified “double” scintigraphic study.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. 147 pancreaticoduodenal resections were performed in the period of 2016–2022. Patients underwent “double” scintigraphic study on days 30 and 90 after surgery. The mean age of the patients was 59.6 years (23–83); the male to female ratio was 1.27:1. All patients underwent gastropancreatoduodenal resection or pancreatoduodenal resection with pylorus preservation. The patients were divided into 3 groups according to the variant of the reconstructive stage of surgery. Group 1 underwent reconstruction according to Child, group 2 – Child reconstruction and interintestinal anastomosis formation according to Brown, the reconstructive stage in group 3 involved Roux-en-Y isolation of the small intestine (Roux-en-Y reconstruction).</p></sec><sec><title>Results</title><p>Results. “Double” scintigraphic study revealed that the best result on the 30th and 90th postoperative days was observed in patients who underwent pancreaticoduodenal resection with pylorus preservation, reconstructive stage according to Child and formation of interintestinal anastomosis after Brown.</p></sec><sec><title>Conclusion</title><p>Conclusion. “Double” scintigraphy is an effective procedure to determine the motility of the stomach, biliodigestive anastomosis and motility of the small intestine.
