Оценивалось влияние на результаты лечения вида предоперационного желчеотведения. Изучены частота и характер послеоперационных желчеистечений, в т.ч. как источника инфекционных осложнений. Проведен анализ микробной флоры отделения ГПБ хирургии за 10 лет.</p></sec><sec><title>Результаты</title><p>Результаты: Выбор варианта дренирования желчных протоков с потенциальным инфицированием не влияли на количество значимых осложнений, в т.ч. инфекционных при стандартном течении послеоперационного периода после ПДР. Ведущий фактор развития интраабдоминальных инфекционных осложнений и основная причина неблагоприятных исходов – наличие панкреатической фистулы.  После резекций печени  наиболее часто из осложнений встречалось послеоперационное желчеистечение - 95 случаев (7,5 %) и достоверно чаще диагностировалось у пациентов после обширных резекций печени.</p><p>При изучении микрофлоры в желчи, полученной из наружных дренажей желчных протоков  и раневом отделяемом преобладала грамотрицательная флора  2/3 случаев, в трети случаев грамположительная флора, менее чем в 5% - грибы.  В дренажном отделяемом лидирующими патогеннами были полирезистентные грамотрицательные микроораганизмы: у большинства штаммов определялась устойчивость к цефалоспоринам, фторхинолонам, пенициллинам, в динамике повыысилась резистентность к карбапенемам.</p></sec><sec><title>Заключение</title><p>Заключение: Бактеробилия у пациентов после дренирования желчевыводящих протоков является существенным фактором, определяющим микробный пейзаж отделения ГПБ хирургии.  Важным предиктором инфекционных осложнений в зоне вмешательства после ПДР является несостоятельность панкреато-дигестивного анастомоза и требует назначения антимикробной терапии с учетом исходной флоры билиарного тракта. Основным  фактором, способствующим развитию послеоперационных инфекционных осложнений после резекции печени является желчеистечение.</p><p>Обширная резекция печени может приводить к развитию сепсисоподобного состояния в раннем послеоперационном периоде, что является предрасполагающим фактором развития инфекционных осложнений.</p></sec></abstract><kwd-group xml:lang="ru"><kwd>инфекционные осложнения</kwd><kwd>резекции печени</kwd><kwd>панкреатодуоденальная резекция</kwd><kwd>дренирование желчных протоков</kwd><kwd>микробный пейзаж</kwd><kwd>антибактериальная терапия.</kwd></kwd-group></article-meta></front><back><ref-list><title>References</title><ref id="cit1"><label>1</label><citation-alternatives><mixed-citation xml:lang="ru">Kokudo T, Uldry E, Demartines N, Halkic N.
