Улучшение результатов лечения при остром билиарном панкреатите у детей с холецисто- и холедохолитиазом.</p></sec><sec><title>   Материал и методы</title><p>   Материал и методы. Проведен ретроспективный анализ обследования и лечения 14 детей 3–17 лет с острым билиарным панкреатитом средней и тяжелой степени. В 11 (78,5 %) наблюдениях срок от начала заболевания до госпитализации составил &lt; 24 ч, в 3 (21,4 %) наблюдениях &gt; 48 ч. Всем больным проводилось комплексное обследование, включавшее клинико-лабораторные и инструментальные методы исследования.</p></sec><sec><title>   Результаты</title><p>   Результаты. Ультразвуковые признаки острого холецистита выявлены у 10 (83,3 %) детей, билиарная гипертензия – у 12 (85,7 %). Лапароскопическая холецистэктомия до 72 ч от начала заболевания проведена 1 (7,14 %) пациенту, 1 ребенку – открытая холецистэктомия с литэкстракцией и дренированием общего желчного протока по Керу. В 1 наблюдении выполнена открытая холецистостомия с дренированием гнойно-некротического парапанкреатита. Эндоскопическая папиллосфинктеротомия, литэкстракция и холангиография выполнены 4 (28,6 %) детям, 1 ребенку дополнительно выполнено стентирование общего желчного протока и протока поджелудочной железы. У всех пациентов диагностирован некротический панкреатит с различными типами парапанкреатита. Дренирование парапанкреатита в МОНИКИ выполнено 5 (35,7 %) детям: 2 (14,3 %) – открытым способом, 2 (14,3 %) – лапароскопическая санация и 1 – чрескожное дренирование под контролем УЗИ.</p></sec><sec><title>   Заключение</title><p>   Заключение. Экстренная эндоскопическая папиллосфинктеротомия с литэкстракцией является методом выбора в устранении билиарной гипертензии при холедохолитиазе. У детей с тяжелым или среднетяжелым острым билиарным панкреатитом предпочтение следует отдавать отсроченной или плановой холецистэктомии. Выбор метода дренирования инфицированного парапанкреатита зависит от распространенности и адекватности санации гнойного процесса.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>   Aim</title><p>   Aim. To improve treatment outcomes for acute biliary pancreatitis in children with cholecysto- and choledocholithiasis.</p></sec><sec><title>   Materials and methods</title><p>   Materials and methods. The authors conducted a retrospective analysis of the examination and treatment of 14 children aged 3–17 years with moderate and severe ABP. In 11 observations (78.5 %) the period from the onset of the disease to hospitalization comprised &lt; 24 hours, in 3 observations (21.4 %) &gt; 48 hours. All patients underwent a comprehensive examination including clinical, laboratory and instrumental diagnostics.</p></sec><sec><title>   Results</title><p>   Results. Ultrasound signs of acute cholecystitis were detected in 10 children (83.3 %), biliary hypertension – in 12 (85.7 %). Laparoscopic cholecystectomy was performed in 1 patient (7.14 %) within 72 hours from the onset of the disease; 1 child underwent open cholecystectomy with lithoextraction and drainage of the common bile duct according to Kerr technique. Open cholecystostomy with drainage of purulent-necrotic parapancreatitis was performed in 1 case. Endoscopic papillosphincterotomy, lithoextraction and cholangiography were performed in 4 children (28.6 %), 1 child additionally underwent stenting of the common bile duct and pancreatic duct. All patients were diagnosed with necrotizing pancreatitis and various types of parapancreatitis. Drainage of parapancreatitis in the Vladimirsky Moscow Regional Research Clinical Institute was performed in 5 (35.7 %) children: 2 (14.3 %) by open technique, in 2 (14.3 %) by laparoscopic sanation and in 1 by US-guided percutaneous drainage.</p></sec><sec><title>   Conclusion</title><p>   Conclusion. Emergency endoscopic papillosphincterotomy with lithoextraction refers to a method of choice for the elimination of biliary hypertension in choledocholithiasis. Delayed or elective cholecystectomy is considered preferable for children with severe or moderately-severe acute biliary pancreatitis.
