Увеличить эффективность оперативного лечения детей с пороками развития и заболеваниями поджелудочной железы с помощью эндоскопических вмешательств.</p></sec><sec><title>   Материал и методы</title><p>   Материал и методы. В исследование включили 39 детей 3–17 лет, которым выполнили 58 эндоскопических вмешательств.</p></sec><sec><title>   Результаты</title><p>   Результаты. Транспапиллярные вмешательства выполнены 14 детям, у всех выявлены аномалии протоков железы. Стеноз проксимальных протоков железы выявлен в 5 наблюдениях – выполнили стентирование.Вирсунголитиаз подтвержден в 4 наблюдениях, аномалия в виде неполной расщепленной железы выявлена у 3 из них. Разделенная поджелудочная железа выявлена у 1 больного, выполнена сфинктеротомия малого сосочка двенадцатиперстной кишки. Трансмуральные вмешательства осуществили в 24 наблюдениях: пункцию – 10 пациентам с интра- или парапанкреатическими кистозными образованиями, дренирование – 14 больным с псевдокистами. Комбинированное транспапиллярное и трансмуральное вмешательство выполнено ребенку с травматическим разрывом железы.</p></sec><sec><title>   Заключение</title><p>   Заключение. Транспапиллярные вмешательства позволяют эффективно устранять гипертензию в протоках поджелудочной железы у детей. Для дренирования панкреатических псевдокист у детей можно применять трансмуральные процедуры.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>   Aim</title><p>   Aim. To increase the efficiency of surgical treatment for children with pancreatic malformations and diseases using endoscopic interventions.</p></sec><sec><title>   Materials and methods</title><p>   Materials and methods. The study enrolled 39 children aged 3–17 years who underwent 58 endoscopic interventions.</p></sec><sec><title>   Results</title><p>   Results. Transpapillary interventions were performed in 14 children with pancreatic duct abnormalities. Stenosis of the proximal pancreatic ducts was revealed in 5 observations – stenting was performed. Lithiasis of Wirsung's duct was confirmed in 4 observations, an abnormality in the form of an incomplete pancreas divisum was revealed in 3 of them. Pancreas divisum was detected in 1 patient, sphincterotomy of the minor duodenal papilla was performed. Transmural interventions were carried out in 24 observations: puncture – in 10 patients with intra- or parapancreatic cystic formations, drainage – in 14 patients with pseudocysts. Combined transpapillary and transmural surgery was performed in a child with traumatic pancreatic disruption.</p></sec><sec><title>   Conclusion</title><p>   Conclusion. Transpapillary interventions prove to effectively eliminate hypertension in the pancreatic ducts in children.
