Оценить эффективность лапароскопических операций при заболеваниях и пороках развития поджелудочной железы у детей.</p></sec><sec><title>   Материал и методы</title><p>   Материал и методы. C 2005 по 2023 гг. выполнены 53 лапароскопические операции. Энуклеация выполнена 16 детям, дистальная резекция поджелудочной железы с сохранением селезенки – 16, корпорокаудальная резекция поджелудочной железы – 4, субтотальная резекция поджелудочной железы – 9, центральная резекция поджелудочной железы – 5, резекция головки с сохранением двенадцатиперстной кишки – 1, панкреатодуоденальная резекция с сохранением привратника – 2 детям. Возраст детей варьировал от 2 мес до 17 лет.</p></sec><sec><title>   Результаты</title><p>   Результаты. Все операции достигли планируемого клинического эффекта. Среднее время энуклеации составило 88,1 ± 47,7 мин, дистальной резекции с сохранением селезенки – 100,3 ± 21 мин, корпорокаудальной резекции – 113,3 ± 28,9 мин, субтотальной резекции – 101 ± 21,7 мин, центральной резекции – 243,3 ± 81,1 мин, резекции головки с сохранением двенадцатиперстной кишки – 235 мин, панкреатодуоденальной резекции с сохранением привратника – лапароскопической и лапароскопически ассистированной – 435 и 415 мин. Интраоперационно кровотечение отмечено в 6 (11,3 %) наблюдениях, в 2 (3,8 %) из них выполнена конверсия. В послеоперационном периоде отмечено 6 (13,2 %) ранних и 8 (15,1 %) поздних осложнений. Летальных исходов не было. Катамнез варьировал от 1 года до 10 лет, рецидивов не отмечено.</p></sec><sec><title>   Заключение</title><p>   Заключение. Очаговые поражения поджелудочной железы у детей – гетерогенная группа заболеваний и пороков развития, требующих различных резекционных вмешательств. Наиболее эффективны лапароскопическая энуклеация и дистальная резекция поджелудочной железы. Целесообразность применения у детей лапароскопического метода для выполнения центральной резекции поджелудочной железы и панкреатодуоденальной резекции требует дальнейшего накопления клинического опыта.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>   Aim</title><p>   Aim. To evaluate the effectiveness of laparoscopic surgeries for diseases and malformations of the pancreas in children.</p></sec><sec><title>   Materials and methods</title><p>   Materials and methods. 53 laparoscopies were performed from 2005 to 2023. Enucleation was performed in 16 children, distal resection of the pancreas with spleen preservation – in 16, corpo-caudal resection of the pancreas – in 4, subtotal resection of the pancreas – in 9, central resection of the pancreas – in 5, head resection with duodenal preservation – in 1, pancreaticoduodenal resection with pyloric preservation – in 2 children. The age of the children ranged from 2 months to 17 years.</p></sec><sec><title>   Results</title><p>   Results. All surgeries have achieved the intended clinical effect. The average time of enucleation accounted for 88.1 ± 47.7 min., distal resection with spleen preservation – 100.3 ± 21 min., corpo-caudal resection – 113.3 ± 28.9min., subtotal resection – 101 ± 21.7 min., central resection – 243.3 ± 81.1 min., head resection with duodenal preservation – 235 min., pancreaticoduodenal resection with pyloric preservation – laparoscopic and laparoscopically assisted – 435 and 415 min. Intraoperative bleeding was noted in 6 cases (11.3 %), in 2 (3.8 %) conversion was performed in 2 cases (3.8 %). The postoperative period revealed 6 (13.2 %) early and 8 (15.1 %) late complications. No fatal outcomes reported. The follow-up ranged from 1 to 10 years, no recurrences reported.</p></sec><sec><title>   Conclusion</title><p>   Conclusion. Focal pancreatic lesions in children refer to heterogeneous diseases and malformations requiring various resection interventions. Laparoscopic enucleation and distal resection of the pancreas are considered as the most effective.
