Цель. Установить, способствует ли операция TIPS, обеспечивающая эффективную портальную декомпрессию, существенному продлению жизни и улучшению ее качества при вынужденном отказе от трансплантации печени у пациентов старше 60 лет с циррозом и сопутствующей патологией.

Материал и методы

Материал и методы. В исследование включены 27 больных в возрасте старше 60 лет с отягощенным коморбидным фоном, подвергшиеся операции TIPS в период 2016–2023 гг. в связи с угрожающими жизни проявлениями осложненной портальной гипертензии цирротического генеза.

Результаты

Результаты. У всех 27 больных, подвергшихся операции TIPS, достигнута эффективная портальная декомпрессия, подтвержденная редукцией градиента портального давления на 55,4 ± 5,2%. Госпитальной и 6-недельной летальности не было. В течение 1 года после шунтирующей операции отмечено 2 (7,4%) летальных исхода.

Заключение

Заключение. Помимо уменьшения риска развития опасных для жизни осложнений, редукция портальной гипертензии при циррозе печени обеспечивает благоприятные условия для терапии сопутствующих заболеваний, ограниченной из опасения декомпенсации печеночной недостаточности. Достигаемая стабилизация коморбидного фона положительно влияет на возможность пересмотра отказа от включения пациента в лист ожидания трансплантации печени.

Aim

Aim. To determine the potential of TIPS that provides effective portal decompression for longevity and quality of life of patients over 60 years with cirrhosis and concomitant pathology when liver transplantation is reasonably denied.

Materials and methods

Materials and methods. The study included 27 patients aged over 60 with comorbidities, who underwent TIPS in 2016–2023 due to life-threatening manifestations of complicated portal hypertension of cirrhotic origin.

Results

Results. TIPS provided effective portal decompression in all 27 patients, confirmed by a 55.4 ± 5.2% reduction in portal pressure gradient. No hospital and 6-week mortality was recorded. 2 deaths (7.4%) were registered within one year after shunt surgery.

Conclusion

Conclusion. In addition to lessening the risk of life-threatening complications, reduction of portal hypertension in liver cirrhosis provides favorable conditions for therapy of concomitant diseases, limited by concerns regarding hepatic decompensation. Due to the achieved stabilization of comorbidities, the denial to include the patient in the waiting list for liver transplantation can be reconsidered.
