Изучить критерии тяжести острого билиарного панкреатита при различных причинах нарушения оттока желчи в двенадцатиперстную кишку, в том числе билиарном сладже, проанализировать результаты лечения больных.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. Ретроспективно анализировали результаты диагностики и лечения 89 больных с доказанной причиной нарушения проходимости общего желчного протока после эндоскопической папиллосфинктеротомии (ЭПСТ). Тяжесть панкреатита оценивали по органной недостаточности, некрозу поджелудочной железы, выраженности холангита. Изучали наличие сладжа в желчном пузыре при УЗИ, папиллита – при эзофагогастродуоденоскопии.</p></sec><sec><title>Результаты</title><p>Результаты. В 1-й группе у 23 больных причиной нарушения оттока желчи был билиарный сладж, во 2-й группе (n = 32) – вклиненный конкремент, в 3-й (n = 34) – холедохолитиаз. Органная недостаточность отмечена у 11 больных 1-й группы, у 13 – 2-й группы и 7 – 3-й группы (р &lt; 0,04). В 1-й группе органная недостаточность наступила на фоне панкреонекроза (n = 7) и острого холангита (n = 4), во 2-й группе у всех 13 больных присутствовал острый холангит (р &lt; 0,05). Доля больных с папиллитом в 1-й и 2-й группах была достоверно больше. Сладж в желчном пузыре выявлен у 14 больных 1-й группы, у 4 – во 2-й группе и у 8 – в 3-й группе (р &lt; 0,001). Назобилиарный дренаж после ЭПСТ применили в 34 наблюдениях, стент протока поджелудочной железы – в 33, инвазивные вмешательства по поводу парапанкреатита – в 9. Умерло 3 (3,4%) больных.</p></sec><sec><title>Заключение</title><p>Заключение. Некроз поджелудочной железы и острый холангит определяли тяжесть острого билиарного панкреатита, что согласуется с концепцией 2 типов болезни – панкреатического и билиарного. Билиарный сладж в желчном протоке наряду с вклиненным конкрементом был причиной обструкции желчного протока у больных с органной недостаточностью. Сладж в желчном пузыре и папиллит при наличии признаков желчной гипертензии позволяют предположить его наличие в общем желчном протоке и рассмотреть показания к ЭПСТ. Выявлены важные закономерности, которые могут быть учтены в протоколах последующих проспективных исследований.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To investigate the severity criteria of acute biliary pancreatitis in various causes of bile outflow obstruction into the duodenum, including biliary sludge, and to analyze the treatment outcomes of patients.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. A retrospective analysis was conducted on the diagnostic and treatment results of 89 patients with a confirmed cause of obstruction of the common bile duct following endoscopic papillosphincterotomy. The severity of pancreatitis was assessed in terms of organ failure, pancreatic necrosis, and the severity of cholangitis. The presence of sludge in the gallbladder was evaluated by means of ultrasound, and papillitis was assessed during esophagogastroduodenoscopy.</p></sec><sec><title>Results</title><p>Results. In Group 1, 23 patients had biliary sludge as the cause of bile outflow obstruction; in Group 2 (n = 32), the cause lied in an impacted stone; and in Group 3 (n = 34), in choledocholithiasis. Organ failure was noted in 11 patients of group 1, in 13 patients of group 2, and in 7 patients of group 3 (p &lt; 0.04). In group 1, organ failure occurred against the background of pancreatic necrosis (n = 7) and acute cholangitis (n = 4); in group 2, all 13 patients exhibited acute cholangitis (p &lt; 0.05). The proportion of patients with papillitis was significantly higher in Groups 1 and 2. Sludge in the gallbladder was detected in 14 patients of Group 1, 4 patients of Group 2, and 8 patients of Group 3 (p &lt; 0.001). Nasobiliary drainage after endoscopic papillosphincterotomy was applied in 34 cases, pancreatic duct stenting in 33 cases, and invasive interventions for peripancreatitis in 9 cases. 3 patients (3.4%) died.</p></sec><sec><title>Conclusion</title><p>Conclusion. Pancreatic necrosis and acute cholangitis determined the severity of acute biliary pancreatitis, which is consistent with the concept of 2 types of disease – pancreatic and biliary. Biliary sludge in the bile duct, alongside impacted stones, caused bile duct obstruction in patients with organ failure. Sludge in the gallbladder and papillitis, along with signs of biliary hypertension, suggest its presence in the common bile duct and indicates the need for endoscopic papillosphincterotomy.
