Продемонстрировать возможности интраоперационной топической диагностики нейроэндокринных опухолей поджелудочной железы с применением УЗИ.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. Проведен анализ оперативного лечения 436 пациентов с нейроэндокринными опухолями поджелудочной железы. В 82 наблюдениях (контрольная группа) интраоперационную ревизию проводили визуально и пальпаторно. На остальных 354 операциях (основная группа), помимо осмотра и пальпации, использовали интраоперационное УЗИ, которое выполняли после мобилизации поджелудочной железы.</p></sec><sec><title>Результаты</title><p>Результаты. В контрольной группе у 15% пациентов во время операции выявить опухоль не удалось. В основной группе чувствительность визуальной и пальпаторной оценки составила 81%, чувствительность интраоперационного УЗИ при лапаротомии – 98,8%, при лапароскопическом доступе – 95%. Разработан метод энуклеации непальпируемых нейроэндокринных опухолей поджелудочной железы под контролем интраоперационного УЗИ.</p></sec><sec><title>Заключение</title><p>Заключение. Оперативные вмешательства при нейроэндокринных опухолях поджелудочной железы следует выполнять с интраоперационным УЗИ. Это позволяет не только выявлять все опухоли в абсолютном большинстве наблюдений, но и определять синтопию с протоковыми структурами и сосудами, что в ряде ситуаций приводит к своевременному изменению хирургической тактики.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To demonstrate the capabilities of intraoperative topical diagnosis of pancreatic neuroendocrine tumors using ultrasound.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. An analysis of surgical treatment in 436 patients with pancreatic neuroendocrine tumors was conducted. In 82 cases (control group), intraoperative exploration was performed through visual inspection and palpation. In the remaining 354 surgeries (main group), intraoperative ultrasound was additionally used following pancreatic mobilization, along with visual inspection and palpation.</p></sec><sec><title>Results</title><p>Results. In the control group, the tumor could not be detected intraoperatively in 15% of cases. In the main group, the sensitivity of visual and palpatory assessment was 81%, while the sensitivity of intraoperative ultrasound was 98.8%during laparotomy and 95% during laparoscopic procedures. An enucleation technique for non-palpable pancreatic neuroendocrine tumors under intraoperative ultrasound guidance was developed.</p></sec><sec><title>Conclusion</title><p>Conclusion. Surgical interventions for pancreatic neuroendocrine tumors should be performed with intraoperative ultrasound.
