Оценить эффективность интеграции ICG-холангиографии в систему обеспечения безопасности при лапароскопической холецистэктомии для предотвращения повреждений внепеченочных желчных протоков за счет повышения точности анатомической навигации и уменьшения риска когнитивных искажений у хирургов.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. Проведен анализ современных литературных источников, посвященных повышению точности интраоперационной идентификации анатомических структур при лапароскопической холецистэктомии с применением флюоресцентной ICG-холангиографии. Осуществляли поэтапную фотофиксацию ключевых этапов 76 оперативных вмешательств, затем – экспертную оценку по балльной шкале, направленную на объективизацию соблюдения критериев хирургической безопасности.</p></sec><sec><title>Результаты</title><p>Результаты. Внедрение концепции критического взгляда на безопасность (CVS) продемонстрировало высокую эффективность в профилактике ятрогенных повреждений. Однако достижение полного CVS не всегда возможно вследствие вариабельной анатомии и спаечных изменений. Методы навигационной хирургии и, в частности, ICG-холангиография являются перспективными инструментами повышения безопасности. Их применение способствует облегчению определения структур гепатобилиарной зоны, воздействуя на ключевые когнитивные механизмы, лежащие в основе хирургических ошибок. Представлен алгоритм типичной холецист эктомии с выполнением критериев CVS и интраоперационной ICG-флюоресцентной холангиографией.</p></sec><sec><title>Заключение</title><p>Заключение. Обязательное соблюдение принципов безопасной лапароскопической холецистэктомии, включая CVS, позволяет существенно увеличить точность идентификации анатомических структур. Использование ICG-холангиографии способствует улучшению обнаружения ключевых анатомических структур, облегчая пространственную ориентацию хирурга и уменьшая вероятность ошибки. Таким образом, ICGхолангиография – ценный инструмент в концепции CVS, который следует рассматривать как важный элемент многоуровневой системы предотвращения повреждений внепеченочных желчных протоков.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To evaluate the effectiveness of integrating ICG-cholangiography into the safety system during laparoscopic cholecystectomy for the prevention of extrahepatic bile duct injuries by improving the accuracy of anatomical navigation and reducing the risk of cognitive biases in surgeons.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. An analysis of current literature was conducted, with a particular focus on improving the accuracy of intraoperative identification of anatomical structures during laparoscopic cholecystectomy using fluorescent ICG-cholangiography. Step-by-step photographic documentation of key stages was performed in 76 surgical procedures, followed by expert evaluation using a scoring system aimed at objectively assessing compliance with surgical safety criteria.</p></sec><sec><title>Results</title><p>Results. The implementation of the Critical View of Safety (CVS) concept demonstrated high effectiveness in preventing iatrogenic injuries. However, achieving complete CVS is not always possible due to anatomical variability and adhesions. Navigation-assisted surgical methods, particularly ICG-cholangiography, represent promising tools for enhancing surgical safety. Their use facilitates the identification of hepatobiliary structures by influencing key cognitive mechanisms underlying surgical errors. An algorithm for a typical cholecystectomy meeting CVS criteria and incorporating intraoperative ICG-fluorescent cholangiography is presented.</p></sec><sec><title>Conclusion</title><p>Conclusion. Strict compliance with the principles of safe laparoscopic cholecystectomy, including the Critical View of Safety (CVS), significantly improves the accuracy of identification of anatomical structures. The use of ICGcholangiography enhances the detection of key anatomical landmarks, facilitates the surgeon’s spatial orientation, and reduces the likelihood of error.
