Систематический обзор и метарегрессионный анализ влияния числа эндоскопических транслюминальных некрсеквестрэктомий на клинические исходы у пациентов с инфицированным отграниченным панкреонекрозом, включая клинический успех и частоту неблагоприятных событий.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. В анализ включены 9 исследований (470 пациентов) за 2020–2025 гг., отобранных по критериям PRISMA. Риск систематических ошибок оценивали с помощью шкалы ROBINS-I. Метарегрессия выполнена в Python 3.9 с применением взвешенного метода наименьших квадратов. Вес исследований рассчитывали как обратную дисперсию стандартной ошибки (1/SE²), скорректированной на размер выборки. Анализировали коэффициент регрессии (β), 95 % доверительные интервалы (95 % ДИ), гетерогенность (I²).</p></sec><sec><title>Результаты</title><p>Результаты. Увеличение числа эндоскопических транслюминальных некрсеквестрэктомий (3–5 процедур) значимо ассоциировалось с ростом клинического успеха (β = 1,904, p = 0,047), достигая эффективности 90–100 %. Однако метарегрессией выявили тенденцию к росту неблагоприятных событий (осложнений) на 9,7 % с каждой дополнительной некрсеквестрэктомией (β = 0,97, p = 0,083), особенно кровотечений (n = 22) и миграции стента (n = 12). Наибольшая частота неблагоприятных событий отмечена при 4 (40,3 %) эндоскопических процедурах, минимальная – при 1,3 (1,5 %). Оптимальный баланс эффективности и безопасности достигнут при 3–4 вмешательствах.</p></sec><sec><title>Заключение</title><p>Заключение. Этапный подход с выполнением 3–4 этапов эндоскопической транслюминальной некрсеквестрэктомии демонстрирует наилучшее соотношение эффективности (95–100 % успеха) и приемлемого риска неблагоприятных событий (12–14 %). Для уменьшения гетерогенности и уточнения оптимального числа мини-инвазивных вмешательств необходимы проспективные исследования с учетом объема (размера) некроза и тяжести панкреонекроза.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To perform a systematic review and meta-regression analysis of the effects of the number of endoscopic transluminal necrosectomies on clinical outcomes in patients with infected walled-off pancreatic necrosis, including clinical success and adverse event rates.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. The analysis included 9 studies (470 patients) of 2020–2025, selected by PRISMA criteria. The risk of bias was assessed using the ROBINS-I tool. Meta-regression was performed in Python 3.9 using a weighted least squares method. The weight of the studies was calculated as the inverse variance of the standard error (1/SE2) adjusted for sample size. The regression coefficient (β), 95% confidence intervals (95% CI), and heterogeneity (I2) were analyzed.</p></sec><sec><title>Results</title><p>Results. The increase in the number of endoscopic transluminal necrosectomies to 3–5 procedures was significantly associated with increased clinical success (β = 1.904, p = 0.047) and resulted in the efficacy of 90–100%. However, meta-regression showed a trend towards an increase of 9.7% in adverse events (complications) with each additional necrosectomy (β = 0.97, p = 0.083), especially bleeding (n = 22) and stent migration (n = 12). The highest and lowest rate of adverse events was observed in 4 (40.3%) and 1.3 (1.5%) endoscopic procedures, respectively. The balance of efficacy and safety is achieved for 3–4 interventions.</p></sec><sec><title>Conclusion</title><p>Conclusion. A bail-out approach with 3–4 endoscopic transluminal necrosectomies shows the best ratio of efficacy (95–100% success) to acceptable risk of adverse events (12–14%).
