Повышение эффективности рентгенохирургического лечения “свежих” повреждений желчных протоков.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. Антеградное рентгенохирургическое вмешательство на желчных протоках в связи с их ятрогенным повреждением во время холецистэктомии или резекции желудка выполнено 12 пациентам. Травма протоков была диагностирована до 5 сут с момента первичной операции. Во всех наблюдениях сначала выполняли наружное дренирование желчных протоков под контролем УЗИ и рентгентелевидения дренажом 8 Fr, в том числе 3 пациентам с нерасширенными желчными протоками.</p></sec><sec><title>Результаты</title><p>Результаты. Билиодигестивный анастомоз с временным сохранением в послеоперационном периоде рентгенхирургически установленного дренажа сформирован 5 пациентам с полным пересечением внепеченочных желчных протоков через 1,5–3 мес после рентгенхирургического желчеотведения. В 5 наблюдениях проведено антеградное рентгенхирургическое восстановление общего желчного протока на наружновнутреннем дренаже. В 2 наблюдениях после антеградного наружного дренирования желчных протоков выполнено ретроградное эндобилиарное стентирование пластиковым стентом. Летальных исходов не было. Срок наблюдения варьировал от 3 мес до 8,5 лет.</p></sec><sec><title>Заключение</title><p>Заключение. При “свежем” (до 5 сут) повреждении желчных протоков и достаточном опыте хирурга реконструктивная операция выполняется с сохранением дооперационно установленного чрескожного дренажа, позволяющего предотвратить несостоятельность билиодигестивного анастомоза в раннем послеоперационном периоде. При отсутствии технической или временной возможности для ранней реконструктивной операции чрескожный чреспеченочный дренаж следует трансформировать в наружновнутренний. Наружновнутреннее дренирование в комбинации с ретроградным эндоскопическим временным стентированием или без него представляется методом выбора в лечении краевых повреждений внепеченочных желчных протоков. При ятрогенном пересечении внепеченочного желчного протока восстановление непрерывности общего желчного протока антеградным рентгенхирургическим методом также технически возможно в виде продолжительного наружновнутреннего дренирования.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To increase efficacy of X-ray surgical treatment of "fresh" bile ducts injuries.</p></sec><sec><title>Material and Methods</title><p>Material and Methods. Antegrade X-ray surgical intervention for iatrogenic bile ducts injuries during cholecystectomy or stomach resection was performed in 12 patients. Bile duct injury has been diagnosed for 5 days after previous surgery. Percutaneous transhepatic biliary drainage was carried out in all cases including 3 patients with non-dilated intrahepatic bile ducts.</p></sec><sec><title>Results</title><p>Results. Biliodigestive anastomosis with temporary preservation of transhepatic cholangiostomy in postoperative period was carried out in 5 patients with full transection of common bile duct in 1.5–3 months after antegrade transhepatic cholangiostomy. Antegrade recovery of common bile duct by the methods of intervention radiology (transhepatic biliary drainage dislocation in duodenum) was performed in 5 patients. Retrograde endoscopic common bile duct stenting using plastic stent was performed in 2 patients after antegrade biliary drainage. There were no deaths after surgery in this series. Follow-up varied from 3 months to 8,5 years.</p></sec><sec><title>Conclusion</title><p>Conclusion. Early (within 5 days) iatrogenic bile duct injuries and sufficient surgeon's experience suppose reconstructive surgery with preservation of percutaneous transhepatic biliary drainage that prevents biliodigestive anastomosis failure in postoperative period. Percutaneous transhepatic biliary drainage should be transformed in antegrade biliodigestive stent in case of absence of technical or temporary possibility for early reconstructive surgery. Antegrade biliodigestive stenting combined with retrograde endoscopic stenting or without it should be preferred in surgical management of tangential iatrogenic bile duct injuries.
