Определить возможность, необходимость и безопасность применения саморасширяющихся покрытых эндобилиарных стентов при доброкачественных билиарных стриктурах, обусловленных хроническим панкреатитом.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. Девяти пациентам с безболевым хроническим панкреатитом, осложненным механической желтухой, поэтапно выполняли чрескожную чреспеченочную холангиостомию дренажом “pigtail” 8 Fr, трансформировали ее во временный транспапиллярный наружновнутренний дренаж. В дальнейшем проводили антеградное эндобилиарное стентирование зоны стриктуры покрытым саморасширяющимся металлическим стентом диаметром 9 мм. Признаков острого панкреатита не было. Для контроля положения стента и его проходимости наружновнутренний дренаж в просвете эндобилиарного стента сохраняли до 2–4 нед.</p></sec><sec><title>Результаты</title><p>Результаты. Технический успех при антеградной установке саморасширяющегося покрытого стента в транспапиллярную позицию достигнут у всех больных. Время полного открытия стента составило 2 сут. УЗИ проводили каждые 2 мес с момента установки стента. Все 9 стентов были успешно и без осложнений извлечены эндоскопически через 6–12 мес. Рецидива механической желтухи не было. Средняя продолжительность безрецидивного наблюдения составила 22,8 мес.</p></sec><sec><title>Заключение</title><p>Заключение. Дилатация зоны стриктуры дистального отдела общего желчного протока до 8–10 мм при раскрытии покрытого стента позволяет уверенно ликвидировать билиарную гипертензию. Расположение стента в этой зоне за 6–9 мес приводит к формированию устойчивого к внешнему воздействию перидуктального каркаса, сохраняющегося и после его эндоскопического извлечения. Миниинвазивное рентгенхирургическое устранение билиарной гипертензии покрытым саморасширяющимся металлическим стентом в транспапиллярной позиции у больных хроническим панкреатитом, основным синдромом заболевания которых является механическая желтуха, может стать альтернативой традиционному хирургическому вмешательству.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To determine the possibility, necessity and safety of self-expanding covered endobiliary stents in benign biliary strictures caused by chronic pancreatitis.Material and methods. Nine patients with painless chronic pancreatitis complicated by obstructive jaundice syndrome were enrolled. Minimally invasive treatment included stage-by-stage percutaneous transhepatic cholangiostomy by pigtail 8Fr drain followed by its transformation to temporary transpapillary external-internal drainage. Then we performed antegrade endobiliary stenting of stricture with self-expanding metal stent 9 mm. There were no symptoms of acute pancreatitis. External-internal drainage in within bile duct lumen has been kept for 2–4 weeks to control position and patency of the stent.</p></sec><sec><title>Results</title><p> Results. Technical success of antegrade deployment of self-expanding covered stent into transpapillary position was achieved in all patients. The time of complete opening of the stent was 2 days. Ultrasonic control every 2 months was applied since stent has been installed. All 9 stents were successfully and uneventfully extracted endoscopically after 6–12 months. There were no cases of recurrent mechanical jaundice. Mean disease-free follow-up was 22.8 months.</p></sec><sec><title>Conclusion</title><p>Conclusion. Covered stent opening followed by stricture dilation up to 8-10 mm within distal common bile duct confidently eliminates biliary hypertension. Stenting of this area is followed by formation of stable periductal carcass within 6–9 months which is preserved even after endoscopic extraction of the stent.
