Панкреатодуоденальная резекция является наиболее распространенной операцией при доброкачественных опухолях головки поджелудочной железы и хроническом панкреатите. Однако большой объем резекции и неудовлетворительное качество жизни больных в отдаленные сроки наблюдения ставят под сомнение целесообразность операции при хроническом панкреатите, доброкачественных опухолях поджелудочной железы и опухолях с низким потенциалом злокачественности. Современной тенденцией является внедрение проксимальной резекции железы с сохранением двенадцатиперстной кишки. В обзоре литературы рассмотрены основные способы выполнения резекции поджелудочной железы с сохранением двенадцатиперстной кишки, результаты вмешательств и их место в хирургической панкреатологии.

Pancreatoduodenectomy is the most common surgery for benign tumors of the pancreatic head and chronic pancreatitis. However, advanced resection and unsatisfactory quality of life in long-term period call into question the propriety of this procedure for benign tumors of the pancreatic head and chronic pancreatitis. Duodenum-sparing proximal pancreatectomy is the current approach. The review is devoted to the main techniques of duodenum-sparing pancreatectomy, outcomes of these procedures and their role in pancreatic surgery.
