Цель: описать первый в России успешный опыт выполнения full-split-трансплантации двум взрослым реципиентам.

Материал и методы

Материал и методы. 26 сентября 2014 г. в ФГБУ ГНЦ ФМБЦ им. А.И. Бурназяна выполнена первая в России успешная ex-situ-split-трансплантация печени от посмертного донора. Донор органа соответствовал критериям UNOS, Liu. Пара реципиентов была подобрана так, чтобы индекс GRWR для трансплантации обеих долей печени был близок к 1. Реципиенты находились в листе ожидания 12 и 4 мес, оба страдали циррозом печени в исходе хронического вирусного гепатита.

Результаты

Результаты. Продолжительность оперативного вмешательства составила 650 и 660 мин. Общее время холодовой ишемии – 510 мин. Функция трансплантатов была первичной у обоих реципиентов. В обоих наблюдениях развилось подтекание желчи – класс B по ISGLS и 3А по Clavien–Dindo. Реципиенты выписаны из стационара через 33 и 34 сут. Продолжительность жизни в настоящее время составляет 68 мес.

Заключение

Заключение. Трансплантация печени от посмертного донора методом “ex situ split” является сложной как с технической, так и с организационной стороны. Она требует значительного опыта и координации между обеспечивающими ее бригадами. Тем не менее операция должна получить более широкое распространение, что позволит улучшить результаты лечения пациентов с заболеваниями печени в терминальной стадии.

Aim

Aim. To present the first successful full-split liver transplantation for two adults recipients in Russia.

Materials and methods

Materials and methods. The first successful full-split liver transplantation for two adults in ex situ way in Russia was made on 26th of September 2014 in the Burnasyan Federal Medical Biophysical Center of FMBA. The deceased donor was inside UNOS, Lee. The GRWR index in both recipients was near 1. The first recipient had been in a waiting list for 1 year, the second for 4 months. Both recipients had got liver cirrhosis in terminal stage.

Results

Results. The surgical procedure length was 650 and 660 min. The overall time of cold ischemia was 510 min. We observed a primary function of each graft. ISGLS B and Clavien-Dindo 3A biliary leak complications were observed in both recipients. Both patients were discharged after 33 and 34 days. Overall survival for this moment is 68 months.

Conclusion

Conclusion. Full-split liver transplantation for two adults in ex situ way seems to be a complicated procedure both from the technical and organizing points of view. It demanding good mastership and coordination between surgical team members. At the same time, that treatment method has to be spread widely to improve treatment of patients with end-stage cirrhosis results.
