Рассмотрен опыт лечения 350 пациентов с гормонально активными образованиями поджелудочной железы. Пяти пациентам с инсулинпродуцирующими опухолями и клинической картиной органического гиперинсулинизма была выполнена микроволновая абляция нейроэндокринных опухолей.Результаты. У всех пациентов с инсулиномами после микроволновой абляции удалось добиться стойкой ремиссии клинических проявлений гиперинсулинизма. В 2 наблюдениях осложнений после манипуляции не было. У 2 больных отмечены осложнения 1–2-й степени по Clavien–Dindo, в 1 наблюдении потребовалась повторная госпитализация (3-я степень по Clavien–Dindo).Заключение. Роль микроволновой абляции при нейроэндокринных опухолях поджелудочной железы до конца не определена. Применение методов локальной деструкции необходимо в отдельных наблюдениях, по строгим показаниям. Это не позволяет анализировать большие серии пациентов и оценить отдаленные результаты лечения. Вместе с тем микроволновая абляция дает возможность эффективно контролировать симптомы гормональной гиперпродукции у пациентов с высоким и крайне высоким риском послеоперационных осложнений</p></abstract><trans-abstract xml:lang="en"><p>Aim: to present the first experience of using of microwave ablation in management of functioning pancreatic neuroendocrine tumors.Material and Methods. We present treatment of 350 patients with hormone-producing pancreatic tumors. Five of them with symptoms of organic hyperinsulinism underwent microwave ablation (MWA) of tumors.Results. Sustained remission of clinical manifestations was achieved in all patients with insulinomas after microwave ablation. There were no complications after manipulation in 2 cases. 2 patients had complications Clavien-Dindo degree 1–2, in 1 case readmission was required (Clavien–Dindo degree 3).Conclusion. The role of microwave ablation in pancreatic neuroendocrine tumors management is not defined.
