Определить безопасность и эффективность рандеву-технологий при холедохолитиазе.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. Ретроспективно и проспективно анализировали результаты применения чрескожного чреспеченочного доступа в желчные протоки и эндоскопического вмешательства в рамках рандеву-технологий при холедохолитиазе у 100 пожилых пациентов. Пациенты были разделены на две группы по 50 человек. У пациентов I группы осуществление рандеву начинали с антеградного этапа, у пациентов II группы – с ретроградного. При анализе результатов учитывали постманипуляционные осложнения и неудачи. Контрольная точка исследования – выявление зависимости частоты осложнений, связанных с манипуляцией, от последовательности этапов рандеву-технологии.</p></sec><sec><title>Результаты</title><p>Результаты. Выявлена статистически достоверная прямая зависимость результатов рандеву-технологий от последовательности их выполнения у пожилых пациентов с холедохолитиазом на фоне механической желтухи. Это позволяет рекомендовать для таких пациентов чрескожный чреспеченочный доступ до этапа эндоскопического вмешательства в рамках рандеву-технологии. Частота осложнений и неудач у пациентов I группы составила 18%, II группы – 52%.</p></sec><sec><title>Заключение</title><p>Заключение. Расширение внутрипеченочных желчных протоков 2–3-го порядка в сочетании с желтухой средней и тяжелой степени является предиктором осложнений при неэффективности эндоскопического пособия. В таких ситуациях превентивное антеградное дренирование билиарного тракта в рамках технологии рандеву позволяет предотвратить холангит, эффективно устранить билиарную гипертензию, уточнить выраженность стеноза большого сосочка антеградной баллонной ревизией, а также обезопасить отсроченное эндоскопическое разрешение билиарной патологии.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. Define the safety and effectiveness of rendezvous technique for choledocholithiasis.</p></sec><sec><title>Material and methods</title><p>Material and methods. We performed a retrospective and prospective analysis of the results in percutaneous transhepatic access to the bile ducts and endoscopic intervention in the rendezvous technique for choledocholithiasis in 100 elderly patients. The patients were divided into 2 groups for 50 people. The implementation of the rendezvous began from the antegrade stage in patients from group I; in patients of group II – from the retrograde stage. The control point of the study is the identification of complications associated with manipulation from the next stages of the rendezvous technique.</p></sec><sec><title>Results</title><p>Results. We revealed a direct dependence of the results in sequence of rendezvous technique for elderly patients with choledocholithiasis and obstructive jaundice. This fact allows recommending percutaneous transhepatic access before endoscopic intervention for these patients with statistical significance. The frequency of complications and failures in patients of group I was 18%, in group II – 52%. This allows for such patients percutaneous transhepatic access before endoscopic intervention in the rendezvous technique. Dilatation of intrahepatic segmental and subsegmental 2 and 3 biliary ducts with moderate and severe obstructive jaundice is a predictor of complications with ineffective endoscopic approach.
