Оценить возможности усовершенствованного метода чрескожного чреспеченочного билиарного дренирования (ЧЧБД) с помощью Y-образной системы катетеров при рецидивирующих рубцовых стриктурах желчных протоков.Материал и методы. Анализировали результаты лечения 11 больных с рецидивирующими рубцовыми стриктурами гепатикоеюноанастомоза (ГЕА), сформированного в связи с повреждением желчных протоков во время лапароскопической холецистэктомии. Рубцовые стриктуры у 5 больных рецидивировали после одной, у 4 – после двух, у 2 – после трех повторных реконструктивных операций. В I группе, состоявшей из 9 больных, длякоррекции стеноза ГЕА была применена общепринятая методика ЧЧБД с помощью одинарного билиарного катетера с постоянным его промыванием. Каждые 3 мес выполняли баллонную дилатацию зоны стриктуры с заменой билиарного дренажа. Во II группе, включавшей 4 больных (в том числе двоих из I группы с рестенозом), была применена Y-образная система катетеров, создающая постоянное растяжение зоны стриктуры.Наружную фиксацию и регулярное промывание этих дренажей не выполняли. Их замену проводили также каждые 3 мес.Результаты. В I группе остаточные явления стеноза, контролируемые по деформации баллонного катетера, сохранялись в среднем 7,3 (6–12) мес. Холангит, обусловленный нарушением проходимости и смещением билиарного дренажа, развился у 4 больных. Общая средняя продолжительность дренирования составила 13,8 (12–17) мес. Время наблюдения после извлечения дренажа варьировало от 4 до 10 лет. Клиническая картина рестеноза развилась у 2 больных через 3 и 4,5 года. Во II группе у всех больных олангиографическиепризнаки стеноза анастомоза отсутствовали уже через 3 мес от начала дренирования с применением Y-образной системы катетеров. Осложнений процедуры не отмечено. Средняя продолжительность дренирования составила 10,5 (9–12) мес. У всех больных отмечено безрецидивное состояние на протяжении 2, 13, 14 и 16 мес соответственно после удаления билиарного дренажа.Заключение. Применение чрескожного чреспеченочного дренирования с использованием Y-образной системы катетеров у ряда больных с неоднократно рецидивирующими рубцовыми стриктурами гепатикоеюноанастомоза может быть эффективной альтернативой хирургическому лечению. По сравнению с традиционным методом дренирования усовершенствованная методика ЧЧБД создает условия для постоянного растяжения рубцовых тканей, сокращает сроки дренирования, уменьшает опасность развития осложнений и характеризуется лучшим качеством жизни больных в процессе лечения.</p></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To evaluate an improved method of percutaneous transhepatic biliary drainage (PTBD) with the Y-type catheter system for patients with post-traumatic recurrent bile duct stricture.Material and Methods. A retrospective analysis examined management of 11 patients with recurrent hepatico-jejunal anastomosis stricture after iatrogenic injury of bile ducts during laparoscopic cholecystectomy. Recurrent stricture was observed in 5 patients after 1 reconstructive biliary intervention, in 4 patients after 2 operations and in 2 patients after 3 operations. 9 patients from group I were treated with a conventional long-term single drainage technique with continuous daily flushing. Repeated balloon dilatation of stricture and biliary drain exchange were performed every</p></sec><sec><title>3 months</title><p>3 months. 4 patients from group II (including 2 patients with unsuccessful conventional treatment from group I) were treated with the Y-type drainage catheter system achieving permanent dilatation of bile duct stricture. An exchange of this drainage system was done once every 3 months without flushing or fixation to the skin.Results. Patients from group I demonstrated the residual biliary stricture confirmed by a deformation of balloon catheter within average period of 7.3 months (6–12 months). Cholangitis due to dislocation and obstruction of the drain was observed in 4 patients. Mean overall drainage time was 13.8 months (12–17 months) with clinical follow-up from 4 to 10 years after drain elimination. Symptoms of recurrent stricture were observed in 2 patients after 3 and 4.5 years.Patients from group II demonstrated complete resolution of cholangiographic signs of stricture within 3 months after PTBD using Y-type catheter system. There were no complications. Mean drainage time was 10.5 months (9–12 months). There were no recurrences in 2, 13, 14 and 16 months after PTBD removal.Conclusion. PTBD with the Y-type catheter system may be an effective alternative to open reconstructive surgery for some patients with multiple recurrent benign bile duct strictures.
