Улучшение результатов лечения при новообразованиях в VII–VIII сегментах печени, прилежащих к основному стволу правой печеночной вены и (или) формирующим ее крупным ветвям.Материал и методы. С 2016 по 2022 г. в 24 наблюдениях выполнена резекция VII и (или) VIII сегментов печени с резекцией правой печеночной вены и формирующих ее ветвей без реконструкции. Показаниями к операциям в 15 наблюдениях считали метастазы колоректального рака в печени, в 6 – гепатоцеллюлярный (холангиоцеллюлярный) рак, в 1 – ангиомиолипому, в 1 – метастазы рака тела матки, в 1 – метастазы забрюшинной неорганной опухоли. В 11 наблюдениях дополнительно выполнили экономные резекции печени.Результаты. Интраоперационных признаков нарушения венозного оттока не было. У 3 больных после операции отмечена примесь желчи в дренажах, в 1 наблюдении сформировался желчный свищ. Других осложнений не выявили. Летальных исходов после операций не было. В позднем послеоперационном периоде не выявлено специфических осложнений, которые можно связать с нарушением венозного оттока по правой печеночной вене.Заключение. Существующие и образующиеся коллатерали могут обеспечить адекватный отток венозной крови в среднюю печеночную вену, короткие печеночные вены после резекции VII и VIII сегментов печени с резекцией ствола правой печеночной вены и формирующих ее ветвей без реконструкции даже при отсутствии нижней правой печеночной вены, а также без полного блока правой печеночной вены до операции.</p></abstract><trans-abstract xml:lang="en"><p>Aim. To improve outcomes of treatment for neoplasms in segments VII-VIII of the liver adjacent to the main stem of the right hepatic vein and/or to its forming large branches.Materials and methods. In the period of 2016–2022, the resection of segments VII and/or VIII of the liver and the right hepatic vein, as well as its forming branches without reconstruction was performed in 24 patients. Surgical indications were liver metastases of colorectal cancer in 15 cases, hepatocellular cancer (cholangiocarcinoma) in six cases, angiomyolipoma in one case, uterine cancer metastases in one case and primary retroperitoneal tumor metastases in one case. Minor liver resections were additionally performed in 11 cases. Results. There were no intraoperative signs of impaired venous drainage. After surgery bile in the drains was detected in three patients, and a biliary fistula was formed in one case. No other complications were identified. No fatalities were registered after surgery. The late postoperative period revealed no specific complications related to a disturbance of venous drainage through the right hepatic vein.Conclusion.
