Пациенты с описанными ранее 14 подтипами эфферентной сосудистой анатомии, классифицированные в 3 типа, распределены в 4 группы в зависимости от числа реконструированных сосудов. Анализировали варианты анатомии вен правой доли печени, время операции и беспеченочного периода, частоту послеоперационных осложнений, объем кровопотери, в том числе в зависимости от числа реконструированных сосудов, а также выживаемость.</p></sec><sec><title>Результаты</title><p>Результаты. Разработаны принципы и выявлены особенности реконструкции эфферентных вен трансплантата. Время оперативного вмешательства, продолжительность беспеченочного периода, частота послеоперационных осложнений были наибольшими при сепаратном типе оттока крови (р &lt; 0,05). Кровопотеря была наибольшей при реконструкции 3 и 4 вен (р &lt; 0,05). Различий между группами 3 и 4 по кровопотере не установлено. Выживаемость в течение 12, 36, 60 и 120 мес составила 84, 83, 81 и 71%, различий между группами нет. </p></sec><sec><title>Заключение</title><p>Заключение. Для успешного выполнения трансплантации правой доли печени необходимо прецизионное картирование венозной анатомии донора, сохранение всех потенциально важных эфферентных сосудов и стремление к максимально полной их реконструкции вне зависимости от сложности эфферентной сосудистой анатомии.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To present the technical features and results of transplantation of the right hepatic lobe from a living donor with various types of efferent venous anatomy.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. 306 liver transplantations from living related donors were performed from 2009 to 2021. Patients with previously described 14 subtypes of efferent vascular anatomy and classified into 3 types, were divided into 4 groups depending on the number of reconstructed vessels. The author analyzed anatomy variants of the right hepatic lobe, duration of surgery and anhepatic period, postoperative morbidity, volume of blood loss, including with allowance for the number of reconstructed vessels, as well as survivability.</p></sec><sec><title>Results</title><p>Results. The study demonstrated the principles and features of the reconstruction of the efferent veins of the graft. Time of surgical intervention, duration of anhepatic period, and postoperative morbidity were greatest with a separate type of blood outflow (p &lt; 0.05) Blood loss was greatest during reconstruction veins 3 and 4 (p &lt; 0.05). No difference in blood loss was detected between groups 3 and 4.
