Experience of cholangio- and pancreatoscopy in various pancreatic diseases

Abstract
Aim. To demonstrate experience in the successful treatment of patients with various pancreatic diseases using retrograde- and antegrade-access cholangio- and pancreatoscopy.
Material and methods. Over 7 years, 90 patients underwent cholangio- and pancreatoscopy: 60 (66.7%) women and 30 (33.3%) men with a mean age of 58 years. Retrograde and antegrade access was performed on 80 (88.9%) and 8 (8.9%) cholangio- and pancreatic examinations, respectively; combined access was performed in 2 (2.2%) patients.
Results. A total of 31 (34.4%) lithotripsy treatments with lithoextraction were performed. In 25 (27.8%) cases, cholangio- and pancreatoscopy were performed for adenoma of the major duodenal papilla with intraductal growth. In 11 (12.2%) cases, a targeted biopsy of ductal tumors was performed. In 21 (23.4%) cases, the obliterating strictures of the common bile duct were successfully recanalized. Two (2.2%) patients had ductal foreign bodies removed.
Conclusion. The use of cholangio- and pancreatoscopy, both retrograde and antegrade, greatly enhances the ability to diagnose bile duct and pancreatic diseases. The use of these technologies both improves diagnostic accuracy and treatment efficiency and avoids extensive traumatic reconstruction interventions with a high risk of complications.
