ДИАГНОСТИКА И ЛЕЧЕНИЕ АУТОИММУННОГО ПАНКРЕАТИТА (ОБЗОР ЛИТЕРАТУРЫ)

Аутоиммунный панкреатит впервые описан в1961 г. как “первичный воспалительный склероз поджелудочной железы”. В последующих сообщениях это заболевание описывали как лимфоплазмоцитарный склерозирующий панкреатит, хронический склерозирующий панкреатит, неалкогольный протоково-деструктивный панкреатит, воспалительная псевдоопухоль поджелудочной железы и др. Концепция аутоиммунного панкреатита была предложена Yoshida и соавт. в1995 г. Проведены многочисленные клинические, серологические, радиологические и патологоанатомические исследования. Для облегчения клинического ведения больных аутоиммунным панкреатитом были введены диагностические критерии, выявлены серологические маркеры и патологические особенности. Также были получены данные, указывающие на то, что аутоиммунный панкреатит является частью новой клинико-патологической нозологической единицы – IgG4-ассоциированной аутоиммунной болезни. Важным остается вопрос хирургического лечения аутоиммунного панкреатита, которое выполняется чаще всего при подозрении злокачественного новообразования поджелудочной железы. Таким образом, аутоиммунный панкреатит представляет собой актуальную проблему современной медицины, которая требует дальнейшего изучения.
